Death Certificate Application
The clerk’s office only maintains records for deaths that occurred
in The City of Wyandotte. In order to obtain a certified copy of a
Death Record you must:
Complete and sign
a Death Certificate Application
2. Provide a copy of a VALID driver’s license or sanctioned state
3. Send check or money order payable to The City
of Wyandotte if ordering by mail. Cash is accepted at the payment
windows. Credit cards are only accepted when applying IN PERSON, not
for applications sent through the mail.
Effective October 1, 2011, the fee for a certified Death Certificate is $20.00 for the first
copy, and $10.00 for each additional copy. Requests for additional
copies must be made on the same date as the initial copy. When
requests for additional copies at a later date are made, the initial
$20.00 fee applies to the first copy requested.
